Work Environment

This position requires the Physical Therapist to operate in a home care environment, traveling to patients' residences throughout the Teaneck area. You will join a compassionate and interdisciplinary team of healthcare professionals committed to excellence in care. The work hours are set from Monday to Friday, from 9 AM to 5 PM, providing a stable and predictable schedule to help maintain a work-life balance that is essential for employee well-being.

Key Responsibilities

  • Conduct thorough physical assessments to devise tailored therapy plans that align with each patient's specific needs.
  • Design and implement progressive therapy programs addressing mobility issues, pain management, balance enhancement, and overcoming functional limitations.
  • Administer hands-on treatments, facilitating therapeutic exercises, gait training sessions, and fall prevention strategies to ensure safety.
  • Assist clients in safely regaining autonomy in daily activities, enhancing their overall quality of life while adhering to best practice guidelines.
  • Educate patients, caregivers, and family members on effective care strategies, exercise routines, and wellness techniques to promote recovery and prevent future health issues.
  • Maintain meticulous records and documentation in compliance with healthcare standards, thereby ensuring continuity of care and facilitating communication with other healthcare providers.
  • Collaborate actively with nurses, caregivers, and other therapy professionals to provide holistic and coordinated care for all clients.
  • Monitor and evaluate patient progress regularly, making necessary adjustments to treatment plans, while keeping the healthcare team and families informed of any changes in patient condition.

Patient Population / Client Focus

This role focuses primarily on providing therapy to individuals within home care settings, including geriatric patients and those with complex health conditions. The ability to connect with an aging population and support their specific rehabilitation needs will be crucial. Your work will directly contribute to improving their mobility, independence, and overall quality of life.

Qualifications

  • Active New Jersey Physical Therapist license is required.
  • Bachelor's degree in Physical Therapy from an accredited institution; Doctorate or Master's degree is highly preferred.
  • A minimum of two years of experience as a Physical Therapist, with priority given to candidates experienced in home care or geriatric settings.
  • Demonstrated clinical expertise, effective communication skills, and the ability to work well in a collaborative environment.
  • CPR and First Aid certification preferred; additional certifications in specialized areas such as geriatrics or neurological rehabilitation are advantageous.
  • A valid driver's license and reliable transportation are required to reach patient locations effectively.

Preferred Experience

  • Experience working specifically with the elderly population, especially in home care or assisted living environments, is strongly preferred.
  • Proficient in using electronic health record (EHR) systems and other relevant therapeutic applications to streamline patient care.

Compensation and Benefits

We offer a competitive salary range between $90,000 and $100,000 per year, commensurate with experience and qualifications.
