Work Environment

This Full-Time Physical Therapist position requires working onsite, primarily in clients' homes. Key Responsibilities

• Conduct comprehensive physical assessments for each patient in their home, developing detailed and individualized therapy plans aimed at addressing mobility issues, pain management, and functional limitations.

• Provide hands-on therapy treatments, including therapeutic exercise, gait training, and fall prevention strategies, ensuring clients can safely navigate their home environments.
• Educate patients and their families about rehabilitation exercises, wellness strategies, and self-care techniques that empower them to maintain independence at home.
• Accurately document patient assessments, treatment plans, and progress updates in compliance with healthcare standards, ensuring continuity of care among all team members involved.
• Work collaboratively with nursing staff and other healthcare providers, participating in integrated care discussions to coordinate patient treatment effectively.
• Regularly monitor patient progress, making necessary adjustments to treatment plans and communicating significant changes in condition to physicians and family members promptly.

Patient Population / Client Focus

The primary client population served by this role includes elderly individuals who require assistance in regaining mobility and independence following illness, surgery, or chronic conditions. Qualifications

• Must possess an active Physical Therapist license in New Jersey.
• Hold at least a Bachelor's degree in Physical Therapy from an accredited program; a Doctorate or Master's degree is preferred.

Compensation and Benefits
