Work Environment

The role of Physical Therapist at Mend Force is primarily an onsite position that involves delivering home care services. You will be providing therapy directly within patients' homes, allowing you to design individualized treatment plans in a comfortable and familiar environment for our clients. This setting not only fosters meaningful interactions but also assists in monitoring patient progress in daily living activities. Your work schedule will be Monday to Friday from 9 AM to 5 PM, promoting a structured yet flexible routine that supports work-life balance.

Key Responsibilities

  • Conduct thorough evaluations of patients' physical capabilities and needs to develop tailored therapy plans that align with their individual goals.
  • Design and implement customized therapy programs aimed at improving mobility, reducing pain, enhancing balance, and addressing functional limitations.
  • Administer hands-on treatments, provide guided therapeutic exercises, facilitate gait training, and devise fall prevention strategies.
  • Assist patients in regaining and maintaining their autonomy by fostering independence in daily activities, such as transferring, ambulating, and self-care.
  • Educate patients, their families, and caregivers about therapeutic exercises, preventive care techniques, and wellness strategies to support ongoing recovery.
  • Document patient progress meticulously to comply with healthcare standards, ensuring continuity of care and effective communication with the healthcare team.
  • Collaborate closely with nurses, aides, and other rehabilitation professionals to deliver holistic care, enhancing overall patient outcomes.
  • Continuously assess and adapt treatment protocols based on patient development and response to therapy, ensuring timely communication with family members and medical professionals regarding changes in condition.

Patient Population / Client Focus

This role primarily focuses on elderly patients receiving home care services. You will be instrumental in addressing the unique challenges individuals face in this demographic, particularly in regaining functional mobility after injuries, surgeries, or chronic illnesses. Our team places a strong emphasis on working collaboratively with patients in their homes to create a supportive environment conducive to healing and independence.

Qualifications

  • Active Physical Therapist license in New Jersey is required.
  • Bachelor's Degree in Physical Therapy or relevant field from an accredited institution.
  • A minimum of 2 years of clinical experience as a Physical Therapist; experience working with geriatric or home care populations is strongly preferred.
  • Proficient clinical knowledge with excellent communication skills that foster empathy, adaptability, and strong teamwork.
  • CPR and First Aid certifications are preferred. Additional specialized certifications in geriatrics or neurological rehabilitation are a plus.
  • A valid driver's license and reliable transportation are essential for traveling to patients' homes.

Preferred Experience

  • Experience with home health therapy programs and knowledge of community resources that support seniors' health and wellbeing.
  • Familiarity with electronic health record (EHR) systems for documenting patient interactions and treatment progress.
  • Previous roles in interdisciplinary teams collaborating on comprehensive care plans.

Compensation and Benefits
