This is an onsite role located at our modern Rehabilitation Center in Union City, NJ, where you will work closely with a team of healthcare professionals and support staff. Key Responsibilities

As a Physical Therapist at Valera, your daily responsibilities will include:

  • Directing and implementing physical therapy treatment plans tailored to each patient's needs.
  • Conducting comprehensive evaluations of patients' physical function using diagnostic tests for muscles, nerves, joints, and functional abilities.
  • Overseeing and mentoring Physical Therapy Assistants in delivering appropriate care in adherence to established therapy plans.
  • Designing and advising on home exercise programs that motivate and empower clients toward greater independence.
  • Administering therapeutic exercises, traction, massage, and physical agents in patient rehabilitation.
  • Evaluating and recommending adjustments for prosthetic and orthotic devices while collaborating with physicians for optimal care solutions.
  • Preparing detailed clinical documentation, including initial evaluations, care plans, progress notes, and discharge summaries following OASIS guidelines.
  • Engaging with interdisciplinary teams in case conferences to develop and enhance patient care strategies.
  • Providing education and resources to patients and their families regarding rehabilitation strategies and physical therapy devices.
  • Continuous documentation and communication with case managers and family members about patient progress and adjustments needed in care plans.

Patient Population / Client Focus

You will primarily work with individuals recovering from injuries, surgeries, or chronic conditions that require rehabilitation services. Your role will focus on fostering physical recovery while considering the emotional and psychological aspects of rehabilitation. By evaluating patients' home environments, you will provide insightful recommendations that facilitate a seamless transition back to daily life.

Qualifications

To succeed in this role, you must possess:

  • A Doctorate in Physical Therapy (DPT) from an accredited institution.
  • A valid state license to practice physical therapy in New Jersey.
  • Current CPR certification along with a valid driver's license and automobile insurance.
  • A strong commitment to delivering high-quality patient care while maintaining integrity and professionalism.
  • Creative problem-solving skills and the ability to collaborate effectively within a team-oriented environment.

Preferred Experience

A minimum of two years of experience working as a physical therapist in an acute care or home care setting is preferred. Familiarity with federal and state regulations concerning physical therapy practices is an asset.
