Job Summary

Manages the care of all patients in the breast health radiology department in conjunction with the radiologist and radiology staff members with the objective of coordinating high quality patient care across the continuum. Guides breast health patients through the health care system by coordinating appointments, developing relationships with patients and service providers, and tracking interventions and outcomes.

Essential Functions

  • Guides women, referred to the Breast Health radiology department, through the healthcare system by helping them access screening mammograms, further diagnostic workups and follow up treatment as needed. 

  • Monitors patients progress; consults with primary physician or designee and other members of the care delivery team as needed; refers to appropriate physician or program when needed. 

  • Conducts patient education; collaborates with patient to establish realistic goals and activities to enhance patient self-management and participation in plan of care.

  • Coordinates referral to Breast MDC for any women diagnosed with breast cancer if acceptable with the referring physician and patient.

  • Facilitates interaction and communication with health care providers and staff. Coordinates the care of the breast cancer patient with the Breast Health Nurse Navigator for continuity of patient care.

  • Maintains collaborative professional working relationship with clinicians and administrative staff necessary to develop and implement a successful cross-continuum care management for women with benign or malignant breast disease.

  • Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

  • Utilizes the nursing process in the delivery of patient care; to include assessment, planning, interventions, evaluation and patient/family teaching and documents appropriately in unit/hospital records.

  • Assumes responsibility for ongoing personal professional growth. Obtains 1 continuing education unit related to breast health or breast cancer yearly as required. Attends meetings, in-services, and educational programs as required.

  • Monitors quality of care delivered on a continual basis; identifies quality issues and integrates process improvement (P.I.) results into practice.

  • Exhibits a high level of professionalism, role models behavior to others in the organization.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

  • Education - Associate degree in Nursing

  • Experience - Two (2) years in Nursing

Required Certifications, Registrations, Licenses

  • Current SC RN License

  • Certified Breast Care Nurse (CBCN) or Oncology Certified Nurse (OCN)- Preferred 

Knowledge, Skills and Abilities

  • Knowledge of disease management principles, practices and processes for breast specialty preferred

  • Ability to maintains required credentials

  • Radiology skills preferred

  • Oncology skills preferred
