Key Responsibilities

  • Conduct comprehensive evaluations of patients by reviewing medical histories, performing detailed assessments, and documenting results to create effective treatment plans tailored to individual needs.
  • Engage patients and their families throughout the evaluation process, explaining findings and involving them in goal-setting to foster a collaborative approach to recovery.
  • Implement treatment protocols with proficiency, utilizing best practices based on patients' clinical presentations and the evolving landscape of healthcare.
  • Collaborate with other healthcare providers to coordinate comprehensive patient care and consult when necessary to optimize treatment strategies.
  • Identify and document any changes in patients' conditions that warrant modification of care plans, ensuring adaptations are made collaboratively with other healthcare professionals.
  • Complete discharge planning and coordinate follow-up care to provide seamless transitions to home or further rehabilitation.
  • Maintain meticulous documentation of patient care, including initial evaluations, daily encounter notes, interim progress summaries, and discharge evaluations.
  • Exhibit effective time management skills to meet productivity expectations while maintaining highest standards of patient care.
  • Participate in departmental planning to establish and meet departmental goals and compliance guidelines.
  • Mentor and train physical therapy students, serving as a clinical instructor when needed, to support the next generation of healthcare providers.

Patient Population / Client Focus

Qualifications

  • A Bachelor's degree in Physical Therapy from an accredited institution.
