The Primary Care Manager RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. The goals of care management are to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality. Clinical Care Managers assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient's functional status and independence. This position is responsible for the planning, overseeing and monitoring of all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT. This position requires travel between multiple locations.

Oregon Registered Nurse License
~ 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Bachelor's Degree Nursing.
