This position is based onsite at our rehabilitation facility, where you will work closely with a team of healthcare professionals. Key Responsibilities

  • Conduct thorough evaluations of patients to assess their physical abilities and limitations, focusing on both subjective complaints and objective findings.
  • Develop and implement personalized treatment plans based on clinical assessments and patients' goals, adapting interventions as necessary to ensure progress.
  • Continuously monitor and reassess patient conditions, documenting any changes in progress notes and SOAP (Subjective, Objective, Assessment, Plan) format using our electronic health records system.
  • Provide education to patients and their families about the recovery process, potential challenges, and techniques to promote physical health.
  • Collaborate with multidisciplinary team members to ensure comprehensive care for clients who may require additional services, actively participating in case discussions.
  • Facilitate telehealth consultations as needed, ensuring patients receive appropriate care regardless of their circumstances.
  • Adhere to all organizational policies and minimum utilization goals, contributing to the overall success of the facility.

Patient Population / Client Focus

Qualifications

  • A Bachelor's degree in Physical Therapy from an accredited program is required; candidates with a Master's or Doctorate in Physical Therapy (DPT) will be prioritized.
  • Must hold current and unrestricted licensure from the Board of Physical Therapy for the state of New Jersey.
  • Willingness to engage with patients both in-person and via telehealth, based on the clinical appropriateness of each case.

Compensation and Benefits
